JDís Comments:  There is no question that using a DOM properly can significantly increase your chances of finding canals.  While I canít say that use of the DOM or Light/magnification is yet the standard of care.  I can say that if you are not using some type of magnification and illumination on maxillary molar teeth, your patient might be better served by someone who does.  Dr. John Stopko (in Scottsdale) published a now famous and classic study showing he went from finding the ML canal 73% of the time with a head lamp and 2.5x loops to 93% with the DOM and experience.

The clinical literature states a relative success from 18-93%.

The purpose of this study was to evaluate the influence of using the dental operating microscope for Text Box: detection of the mesiolingual canal (aka MB2) orifice compared with unaided vision (no loops or head lamps).

Using a clinical simulation model system, they  used extracted maxillary molars in a dentoform and placed them into mannequins.  After rubber dam placement and preparation of standard access 2 attempts were made to locate the ML canal with unaided vision.  A DOM was used to examine the access and locate the ML canal if possible.  Finally, all teeth were removed, sectioned, stained, and evaluated with the DOM for actual presence of a ML canal.

This study evaluated the Root ZX with different concentrations of NaOCl in a bench top model.  
Text Box: NaOCl concentrations of 5.25%, 2.65%, 1.00% and 0.5 %  were injected into root canals of the extracted teeth with distilled water used as a control.

After measuring the root lengths with the Root ZX, Text Box: the files were locked into place.

There were statistically no differences between the groups when evaluating the distance from the file tip to apical foramen.
